For First-Time Claimants
If you have never used your VA education benefits before, here are the forms you will need to establish a record in your college’s VA Office:
- DCCCD VA Data Sheet (2011-2012)
- DCCCD Certificate of Understanding
- VA Application— signed confirmation page from the Veterans On-line Application Web site (VONAPP)
- Use VA Application 22-1990 if you were on active duty or a reservist, or VA 22-5490 if you are a dependent.
- For new 1606s (Selected Reserve) we also need a Notice of Basic Eligibility, NOBE, from the Reserves.
- Official Degree Plan - Certifications will be done once the transcripts have been evaluated and the official degree plan has been received from the Administrations Office or a parent letter has been received from your main college / university. The degree plan request form is currently available in the Admissions and FA/VA Offices. Transcripts must be turned in to the Admissions Office. Click here For Degree Plan Request Form. For new DCCCD students, new Degree plans will not be printed to 2 weeks after school starts.
- Please submit a copy of your Certificate of Eligibility to our office once you have received it in the mail from the VA Regional Office.
For Transfer Students
If you are a transfer student and you have used your VA education benefits at another college or university, here are the forms you will need to establish a record with your DCCCD college’s VA Office:
- DCCCD VA Data Sheet (2011-2012)
- DCCCD Certificate of Understanding

- Tuition Account Summary (eConnect - login required) — This is submitted every semester in which you want to claim your education benefits.

For Continuing VA Students
If you are a returning VA student who needs to be certified for the next term:
- *NEW* Certification request form: This is a new form for all students requesting VA benefits. This is required for each term along with the Tuition Account Summary for the same term. Effective: July 2011.
- Submit your Tuition Account Summary (eConnect - login required). This is submitted every semester in which you want to claim your education benefits.
- If you are changing or have changed your degree plan, please submit the new official degree plan and a VA request for change of program form (VA Application 22-1995 if you were on active duty or a Reservist, or VA 22-5495 if you are a dependent).
- Ensure the Admissions Office has your current information. E-mail address and phone number changes can be made online at eConnect (login required).
