Hazlewood Students

All VA students utilizing the Hazelwood Exemption Act, please contact the VA Regional Office in Muskogee, Oklahoma at 1-888-GI-Bill-1 or 1-888-442-4551 or www. gibill.va.gov to obtain an updates VA Education Benefits Letter. Please remember that a new Hazelwood Act Exemption Form is required for each semester that you plan to use the exemption.

Veteran students applying for the Hazlewood Act for the first time must submit the following:

  • DD214 - Member 4 copy or long form
  • Letter for the DVA verifying educational benefits have been exhausted. (Students can contact the Regional Office in Muskogee , OK. at 1-888-442-4551 ).
  • A Hazlewood Act Exemption Form. ( HE - V )
  • Receipt verifying cost of the tuition. ( Tuition Account Summary or a copy of your CE sign up sheet completed )
  • Can not be in default of any student loan.

Veteran's Dependent students applying for the Hazlewood Act for the first time must submit the following:

  • Parent's DD214 - Member 4 copy or long form
  • Letter for the DVA verifying educational benefits have been exhausted. (Students can contact the Regional Office in Muskogee , OK. at 1-888-442-4551 ).
  • A Hazlewood Act Exemption Form. ( HE - D )
  • Receipt verifying cost of the tuition. ( Tuition Account Summary or a copy of your CE sign up sheet completed )
  • Can not be in default of any student loan.

Prior Recipients of the Hazlewood Act Exemption at another Institution:

  • Letter for the DVA verifying educational benefits have been exhausted. (Students can contact the Regional Office in Muskogee , OK . at 1-888-442-4551 ).
  • A Hazlewood Act Exemption Form. ( HE - P )
  • Receipt verifying cost of the tuition. ( Tuition Account Summary or a copy of your CE sign up sheet completed )
  • Can not be in default of any student loan.

What does Hazlewood pay for?

  • Hazlewood pays for the cost of the course(s) for non-credit classes. Each course must be state funded.
  • Hazlewood pays for the cost of the course(s) for up to 150 credit hours attempted since Fall 1995.
