I understand that the tutor will help me to the best of his/her ability and that I am responsible for the grades I receive in the class(es). I also confirm the fact that I am currently enrolled in and actively attending the course(s) for which I am requesting tutoring. It would be beneficial for me to inform my instructor that I am receiving tutoring. I will discuss any academic problems, concerns, and/or tutor “no show” with the Tutor Coordinator.
Note: It is the student’s responsibility to notify the tutor/ or tutoring coordinator if he/she cannot make the tutoring session. Once a student has missed 3 sessions without notice, his/her time slot will be given to another student.
