Individual Entry Form
Individual entries for team events will be placed on an existing team roster or a new team will be formed using available individual entries.
***By signing up for an Intramural/Recreation Activity your picture may be taken and used to be put on the website or Intramural Literature. No profits will be made off of your picture if used.
